Great focus. One item that wasn't mentioned but should be about meeting (which I agree most could be handled more effectively) is the "buy-in". It is some what easier to get someone's "buy-in" to your needs their resources if you are in front of them.

Otherwise, the level of importance tends to be less and with everything being "high-importance" how do you prioritize my request with her request.

Check out Death By Meeting by Patrick Lencioni (not sure on spelling of last name). It really changed how we meet in our organization. The premise is that we all hate meetings, but meetings are where everything important happens-deals are brokered, new strategies decided, etc.
